Exstrophy of the bladder is a complex disorder that occurs during fetal development and means that the bladder is essentially inside-out and exposed on the outside of the abdomen. This condition is usually associated with epispadias (exstrophy-epispadias complex), where everted bladder is associated with shortened and upwardly curved penis with dorsal urethral plate insted of normal urethra - in males; or separated hemiclitoris, narrow vaginal opening, wide labia, and short urethra – in females. Since the bladder is exposed to the outside of the body, urine constantly trickles onto the skin causing local irritation.
Real cause of this rare disorder is unknown. This anomaly may occur in varying degrees from mild to severe. It is usually associated with widened pubic bones, outwardly rotated legs and abnormally-shaped abdominal muscles. Surgical correction of this complex anomaly is very demanding and can be done as a one or multi staged procedure, depending on the severity of the disorder and age of the patient. Surgical procedures include closing of the bladder, reconstruction of the bladder neck, reconstruction of the penis by total disassembly technique and approximation of the pelvic rim if neccesery. Main goals of the surgery are to preserve kidney function, create continent bladder, create as normal appearance of extrenal genitalia as possible.
