Preoperative appearance

Very long urethra is created using urethral plate, both labia minora and clitoral hairless skin

Scrotoplasty is finished. Perineum is created as in male. New urethra is created

MLD phalloplasty is performed. Neophallus with long neurovascular pedicle is created

Result after surgery

New urethral opening is placed in distal part of the neophallus

Second stage included implantation of malleable penile prosthesis

Distal urethra is created using buccal mucosa graft
